Aircraft departed orh airport IFR to pwm. Aircraft climbed to 5000 ft, never seen on radar. (Radar data does not show any targets.) entered center airspace without a handoff. Orh airport has limited radar coverage from bdl. Aircraft was on wrong code.

Narrative: ACFT DEPARTED ORH ARPT IFR TO PWM. ACFT CLBED TO 5000 FT, NEVER SEEN ON RADAR. (RADAR DATA DOES NOT SHOW ANY TARGETS.) ENTERED CTR AIRSPACE WITHOUT A HDOF. ORH ARPT HAS LIMITED RADAR COVERAGE FROM BDL. ACFT WAS ON WRONG CODE.
