Narrative:
A hydraulic leak was discovered on the lower surface of the left wing shortly before scheduled departure. I notified my lead. We replaced #3 slat actuator per the maintenance manual due to a leak at the retract restrictor fitting. Since the fitting had unthreaded from the trunnion, we were concerned how it was supposed to be retained. A search of the ipc and maintenance manual showed no parts were missing. The new actuator was installed per the maintenance manual and the plane was returned to service. Several days later, I learned that there should have been an anti-rotation fitting. I finally did find this in the illustrated parts catalog, but not in the maintenance manual. I notified my lead and maintenance coordinator to have the problem corrected on the aircraft. I have submitted a request to ensure that installing the anti-rotation fitting is specified in the maintenance manual.
Original NASA ASRS Text
Title: A B727-200 WAS DISPATCHED IN NON COMPLIANCE AFTER #3 SLAT ACTUATOR WAS REPLACED WITH NO ANTI-ROTATION DEVICE INSTALLED ON THE ACTUATOR TRUNNION.
Narrative: A HYD LEAK WAS DISCOVERED ON THE LOWER SURFACE OF THE L WING SHORTLY BEFORE SCHEDULED DEP. I NOTIFIED MY LEAD. WE REPLACED #3 SLAT ACTUATOR PER THE MAINT MANUAL DUE TO A LEAK AT THE RETRACT RESTRICTOR FITTING. SINCE THE FITTING HAD UNTHREADED FROM THE TRUNNION, WE WERE CONCERNED HOW IT WAS SUPPOSED TO BE RETAINED. A SEARCH OF THE IPC AND MAINT MANUAL SHOWED NO PARTS WERE MISSING. THE NEW ACTUATOR WAS INSTALLED PER THE MAINT MANUAL AND THE PLANE WAS RETURNED TO SVC. SEVERAL DAYS LATER, I LEARNED THAT THERE SHOULD HAVE BEEN AN ANTI-ROTATION FITTING. I FINALLY DID FIND THIS IN THE ILLUSTRATED PARTS CATALOG, BUT NOT IN THE MAINT MANUAL. I NOTIFIED MY LEAD AND MAINT COORDINATOR TO HAVE THE PROB CORRECTED ON THE ACFT. I HAVE SUBMITTED A REQUEST TO ENSURE THAT INSTALLING THE ANTI-ROTATION FITTING IS SPECIFIED IN THE MAINT MANUAL.
