Narrative:
We operated an aircraft where a deferred maintenance item was not properly completed aircraft had right autoplt disconnecting in a turn. Deferred right autoplt back drive, and the AFDS right back drive circuit breaker should have been pulled and collared by maintenance at the time of deferral. This circuit breaker on cockpit overhead panel. Several crews operated the aircraft with this circuit breaker not pulled. Our placard on the deferred item list only gave us flight restrs, not the need to pull the circuit breaker. This information was only on the MEL for the deferred item. The MEL is not normally accessible to the flight crew. My recommendation is that any deferred item that is to be rendered inoperative by a circuit breaker or switch be accessible to the flight crew be listed on our deferral placard. Then the crew can also check that the deferral was completed properly.

Narrative: WE OPERATED AN ACFT WHERE A DEFERRED MAINT ITEM WAS NOT PROPERLY COMPLETED ACFT HAD R AUTOPLT DISCONNECTING IN A TURN. DEFERRED R AUTOPLT BACK DRIVE, AND THE AFDS R BACK DRIVE CIRCUIT BREAKER SHOULD HAVE BEEN PULLED AND COLLARED BY MAINT AT THE TIME OF DEFERRAL. THIS CIRCUIT BREAKER ON COCKPIT OVERHEAD PANEL. SEVERAL CREWS OPERATED THE ACFT WITH THIS CIRCUIT BREAKER NOT PULLED. OUR PLACARD ON THE DEFERRED ITEM LIST ONLY GAVE US FLT RESTRS, NOT THE NEED TO PULL THE CIRCUIT BREAKER. THIS INFO WAS ONLY ON THE MEL FOR THE DEFERRED ITEM. THE MEL IS NOT NORMALLY ACCESSIBLE TO THE FLC. MY RECOMMENDATION IS THAT ANY DEFERRED ITEM THAT IS TO BE RENDERED INOP BY A CIRCUIT BREAKER OR SWITCH BE ACCESSIBLE TO THE FLC BE LISTED ON OUR DEFERRAL PLACARD. THEN THE CREW CAN ALSO CHK THAT THE DEFERRAL WAS COMPLETED PROPERLY.
