Narrative:

En route from phx to oak at approximately 50 NM northeast of lax at FL350, the cabin pressurization horn began to sound. The cabin had reached 10500 ft and climbing at 400-500 FPM. I asked for an immediate descent, called for the checklist. We ran the checklist to no avail. I spoke with dispatch, lax maintenance, lax operations, set up for a divert to lax. Descending through FL180, the cabin stabilized and started to descend. Reaching 12000 ft, we had a normal approach and landing in lax. I left the aircraft without filling out the logbook. The next day the chief pilot informed me that I had forgot to make an entry in the logbook.

Title: A B737-500 IN CRUISE AT FL350 DIVERTED DUE TO LOSS OF PRESSURIZATION BUT MADE NO LOGBOOK ENTRY ON PRESSURIZATION FAILURE.

Narrative: ENRTE FROM PHX TO OAK AT APPROX 50 NM NE OF LAX AT FL350, THE CABIN PRESSURIZATION HORN BEGAN TO SOUND. THE CABIN HAD REACHED 10500 FT AND CLBING AT 400-500 FPM. I ASKED FOR AN IMMEDIATE DSCNT, CALLED FOR THE CHKLIST. WE RAN THE CHKLIST TO NO AVAIL. I SPOKE WITH DISPATCH, LAX MAINT, LAX OPS, SET UP FOR A DIVERT TO LAX. DSNDING THROUGH FL180, THE CABIN STABILIZED AND STARTED TO DSND. REACHING 12000 FT, WE HAD A NORMAL APCH AND LNDG IN LAX. I LEFT THE ACFT WITHOUT FILLING OUT THE LOGBOOK. THE NEXT DAY THE CHIEF PLT INFORMED ME THAT I HAD FORGOT TO MAKE AN ENTRY IN THE LOGBOOK.
