Narrative:
Before I started my walk around, I checked the logbook and noticed there was no current maintenance release document. I made mental note to inform the captain and then proceeded to do the walk around. Upon returning to the cockpit, I performed the remainder of my preflight duties. There were several issues that arose with the flight attendants and I failed to advise the captain of the missing maintenance release document. We pushed back from the gate, got the salute and proceeded to runway. Approximately 45 mins into the flight, I started to complete the logbook and remembered that the maintenance release document wasn't present. I informed the captain at that time 1 factor that lead to the problem was a borrowed part form in the maintenance release document slot in the logbook.

Title: AN A320 FO ADMITTED THAT HE FORGOT TO GIVE THE CAPT A CURRENT MAINT DISCREPANCY FORM UNTIL AFTER TKOF FROM ORD.
Narrative: BEFORE I STARTED MY WALK AROUND, I CHECKED THE LOGBOOK AND NOTICED THERE WAS NO CURRENT MAINT RELEASE DOCUMENT. I MADE MENTAL NOTE TO INFORM THE CAPT AND THEN PROCEEDED TO DO THE WALK AROUND. UPON RETURNING TO THE COCKPIT, I PERFORMED THE REMAINDER OF MY PREFLT DUTIES.
