Narrative:

A pilot had reported to maintenance control that he had found a cut in r-hand outboard main wheel. I inspected the main wheel and that the cut was well into cord. As per our standards, we are supposed to change the tire. I informed control of what was going on. They (maintenance control) had told that they would put a maintenance hold on aircraft so that I would have enough time to change tire. In doing so, I looked up torque valves and got my tools ready. I started the tire change process and was also trying to be quick about things. I removed air pressure from old tire, pulled dust cap and axle nut off when I broke the torque of axle nut. I then used the socket and my hand to remove nut. When it was off, I removed tire and put on new tire. That is what I can clearly recall. Resulting actions, I had forgot to put on axle nut, nut which holds wheel assembly on axle. It was a total accident resulting from working too fast.

Title: A CANADAIR CL65 WAS DISPATCHED IN NON COMPLIANCE WITH THE R OUTBOARD WHEEL AND TIRE ASSEMBLY MISSING THE AXLE NUT.

Narrative: A PLT HAD RPTED TO MAINT CTL THAT HE HAD FOUND A CUT IN R-HAND OUTBOARD MAIN WHEEL. I INSPECTED THE MAIN WHEEL AND THAT THE CUT WAS WELL INTO CORD. AS PER OUR STANDARDS, WE ARE SUPPOSED TO CHANGE THE TIRE. I INFORMED CTL OF WHAT WAS GOING ON. THEY (MAINT CTL) HAD TOLD THAT THEY WOULD PUT A MAINT HOLD ON ACFT SO THAT I WOULD HAVE ENOUGH TIME TO CHANGE TIRE. IN DOING SO, I LOOKED UP TORQUE VALVES AND GOT MY TOOLS READY. I STARTED THE TIRE CHANGE PROCESS AND WAS ALSO TRYING TO BE QUICK ABOUT THINGS. I REMOVED AIR PRESSURE FROM OLD TIRE, PULLED DUST CAP AND AXLE NUT OFF WHEN I BROKE THE TORQUE OF AXLE NUT. I THEN USED THE SOCKET AND MY HAND TO REMOVE NUT. WHEN IT WAS OFF, I REMOVED TIRE AND PUT ON NEW TIRE. THAT IS WHAT I CAN CLRLY RECALL. RESULTING ACTIONS, I HAD FORGOT TO PUT ON AXLE NUT, NUT WHICH HOLDS WHEEL ASSEMBLY ON AXLE. IT WAS A TOTAL ACCIDENT RESULTING FROM WORKING TOO FAST.
