Narrative:
At XA30 am local time I was on a king air 200 as PIC, however, I was 'PNF' at that time. At that hour, caldwell (cdw) tower was closed and I received my IFR clearance from new york TRACON 15 mins prior. The clearance was to depart runway 22 with a left turn to 180 degrees heading. While taxiing, the left seat PF taxied to runway 4 and proceeded to take off. Before I knew what had happened, I realized we were airborne and climbing out of 1200 ft AGL. I was very sick the night before and subsequently got very little sleep. I was feeling too sick to even leave my home but I went to work for fear of losing my job. With all of the pilots currently unemployed, my employer has been telling me that if I don't fly, he will find someone else who will. Never again, will I attempt to fly if I don't feel 100% well.

Title: A BE200 PART 135 FLT DEPARTS FROM A WRONG RWY, USING RWY 4 FOR TKOF VERSUS RWY 22 AT CDW, NJ.
Narrative: AT XA30 AM LCL TIME I WAS ON A KING AIR 200 AS PIC, HOWEVER, I WAS 'PNF' AT THAT TIME. AT THAT HR, CALDWELL (CDW) TWR WAS CLOSED AND I RECEIVED MY IFR CLRNC FROM NEW YORK TRACON 15 MINS PRIOR. THE CLRNC WAS TO DEPART RWY 22 WITH A L TURN TO 180 DEGS HDG. WHILE TAXIING, THE L SEAT PF TAXIED TO RWY 4 AND PROCEEDED TO TAKE OFF. BEFORE I KNEW WHAT HAD HAPPENED, I REALIZED WE WERE AIRBORNE AND CLBING OUT OF 1200 FT AGL. I WAS VERY SICK THE NIGHT BEFORE AND SUBSEQUENTLY GOT VERY LITTLE SLEEP. I WAS FEELING TOO SICK TO EVEN LEAVE MY HOME BUT I WENT TO WORK FOR FEAR OF LOSING MY JOB. WITH ALL OF THE PLTS CURRENTLY UNEMPLOYED, MY EMPLOYER HAS BEEN TELLING ME THAT IF I DON'T FLY, HE WILL FIND SOMEONE ELSE WHO WILL. NEVER AGAIN, WILL I ATTEMPT TO FLY IF I DON'T FEEL 100% WELL.
