Narrative:
We were cleared for the ostia 5D departure with bolsena sa transition from runway 33 at ciampino airport south of rome, italy. Copies are enclosed. The departure has several altitude restrs on it. They were all properly programmed in the FMC. Both pilots checked the departure and route and verbally briefed it. We do an alternative noise abatement procedure for takeoff at european airports since many are noise sensitive. This procedure calls for a climb at takeoff power and V2 +15 speed to 1500 ft AGL, at 1500 ft AGL we select VNAV which causes a power reduction to climb power, then climb with takeoff flaps at V2 +15 KTS to 3000 ft AGL. Upon reaching 3000 ft AGL, we then start retracting flaps and accelerating. This procedure delays selecting VNAV to a higher altitude than normal. The practice is to set highest altitude cleared to in altitude window. In our case, this was 5000 ft. On departure, we made the first restr of crossing cia NDB at or above 2000 ft. The aircraft was light and was climbing very fast. I was a little slow selecting VNAV on the noise abatement departure. It was very windy and turbulent. VNAV didn't engage fast enough to catch the 3000 ft restr at magli intersection and the airplane climbed as high as 4000 ft before VNAV tried to correct. The controller caught the excursion and asked us to confirm our departure assigned. He then cleared us to 5000 ft. The departures out of ciampino and rome are complicated, and even though the FMC was programmed correctly, I think it best to set the lowest altitude restr (ie, 3000 ft) in case VNAV is not engaged in time due to the alternative noise procedure. Also, this crew was coming off a 12 hour rest period where they had been late arriving in rome in the morning and had to situation in traffic to and from the hotel for 1HR 15 mins each way. We were fatigued and I feel that played a part. The FAA needs to stop counting transportation to the hotel or from the hotel as part of our rest.

THIS PROC CALLS FOR A CLB AT TKOF PWR AND V2 +15 SPD TO 1500 FT AGL, AT 1500 FT AGL WE SELECT VNAV WHICH CAUSES A PWR REDUCTION TO CLB PWR, THEN CLB WITH TKOF FLAPS AT V2 +15 KTS TO 3000 FT AGL. VNAV DIDN'T ENGAGE FAST ENOUGH TO CATCH THE 3000 FT RESTR AT MAGLI INTXN AND THE AIRPLANE CLBED AS HIGH AS 4000 FT BEFORE VNAV TRIED TO CORRECT. THE CTLR CAUGHT THE EXCURSION AND ASKED US TO CONFIRM OUR DEP ASSIGNED. HE THEN CLRED US TO 5000 FT. THE DEPS OUT OF CIAMPINO AND ROME ARE COMPLICATED, AND EVEN THOUGH THE FMC WAS PROGRAMMED CORRECTLY, I THINK IT BEST TO SET THE LOWEST ALT RESTR (IE, 3000 FT) IN CASE VNAV IS NOT ENGAGED IN TIME DUE TO THE ALTERNATIVE NOISE PROC. ALSO, THIS CREW WAS COMING OFF A 12 HR REST PERIOD WHERE THEY HAD BEEN LATE ARRIVING IN ROME IN THE MORNING AND HAD TO SIT IN TFC TO AND FROM THE HOTEL FOR 1HR 15 MINS EACH WAY. WE WERE FATIGUED AND I FEEL THAT PLAYED A PART. THE FAA NEEDS TO STOP COUNTING TRANSPORTATION TO THE HOTEL OR FROM THE HOTEL AS PART OF OUR REST.
