Narrative:
Flying from miami to tist we received a clearance from mia ATC to go direct to punta and then tist. We queried mia, since this was not on our route of flight. He verified this, and we selected cdo VOR (punta) and then tist. Approximately 40 mins later, mia mentioned that we were not heading to punta, so we asked for direct tist. They gave that to us. We looked on the fix page of the FMC, and there was a punta waypoint at N1819.9 W06506.6. We selected this and found it only 5 miles west of tist. We assumed this was the originally requested clearance. We then tried to find this fix on our charts. Not on the high altitude charts for the caribbean. Not on the chart for sju, not on any of the apches or sids/stars for tist. There is no record of seeing this fix anywhere that we have publications of, except in the FMC, we noticed that there is, besides this waypoint and cdo, another ADF in the caribbean with the same name. This should be corrected. Callback conversation with reporter revealed the following information: reporter had not received any company feedback. Additional information: sju IFSS advised that 'punta intersection' had been changed 'a while back' to 'kutho intersection', IAF to ILS runway 10, tist airport, because of caribbean area name commonality, especially with punta cana and punta cauedo airports in the mdcs fir/uir.

Title: ZMA CTLR QUESTIONS B757 FLC ISSUED RTE DIRECT PUNTA INTXN AND IS ADVISED OF MULTIPLE FMS FIX NAMES. PROB RESOLVED WHEN CLRED DIRECT TIST ARPT.
Narrative: FLYING FROM MIAMI TO TIST WE RECEIVED A CLRNC FROM MIA ATC TO GO DIRECT TO PUNTA AND THEN TIST. WE QUERIED MIA, SINCE THIS WAS NOT ON OUR RTE OF FLT. HE VERIFIED THIS, AND WE SELECTED CDO VOR (PUNTA) AND THEN TIST. APPROX 40 MINS LATER, MIA MENTIONED THAT WE WERE NOT HEADING TO PUNTA, SO WE ASKED FOR DIRECT TIST. THEY GAVE THAT TO US. WE LOOKED ON THE FIX PAGE OF THE FMC, AND THERE WAS A PUNTA WAYPOINT AT N1819.9 W06506.6. WE SELECTED THIS AND FOUND IT ONLY 5 MILES W OF TIST. WE ASSUMED THIS WAS THE ORIGINALLY REQUESTED CLRNC. WE THEN TRIED TO FIND THIS FIX ON OUR CHARTS. NOT ON THE HIGH ALT CHARTS FOR THE CARIBBEAN. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: RPTR HAD NOT RECEIVED ANY COMPANY FEEDBACK. ADDITIONAL INFO: SJU IFSS ADVISED THAT 'PUNTA INTXN' HAD BEEN CHANGED 'A WHILE BACK' TO 'KUTHO INTXN', IAF TO ILS RWY 10, TIST ARPT, BECAUSE OF CARIBBEAN AREA NAME COMMONALITY, ESPECIALLY WITH PUNTA CANA AND PUNTA CAUEDO ARPTS IN THE MDCS FIR/UIR.
