Narrative:

I was informed by phone on feb/mon/02 by senior check airman that we had flown 1 leg 2 weeks ago with an airworthiness release that was not signed off properly. All our fom states on the subject is that we insure the document is on board -- nothing saying to ensure it is signed. Fom also states maintenance is responsible for its currency. Gray area at best. Maintenance department may self-disclose event to FAA. At any rate, myself and captain are attending special purpose training to remedy situation.

MAINT DEPT MAY SELF-DISCLOSE EVENT TO FAA. AT ANY RATE, MYSELF AND CAPT ARE ATTENDING SPECIAL PURPOSE TRAINING TO REMEDY SIT.
