Narrative:

Misplaced logbook. Flight was at the gate in lax. Maintenance came out to replace the #1 navigation receiver. They fixed the problem and handed the forms to myself. I looked over the write-up and was in the process of reviewing the forms when a jump seat rider came down the jetway. I set the forms down to speak with the jump seater and to check his credentials. The jump seater boarded followed by myself, with the forms sitting on the counter. En route to phx I realized that the forms were not with the aircraft. The forms were put on a flight to phx 1/2 hour behind us and we continued on from phx with the forms onboard. What I believe caused the event: I got distracted with checking the jump seater's credentials. I was just getting ready to go to the cockpit with the logbook in hand when he came up to me. I set the logbook down and that's where it stayed.
