Narrative:
Korry 1 arrival to lga. This is a new arrival to lga which I had never flown. Our commercial chart page is a fold-out, the right portion of which I had folded under so it would fit on my map clip on the yoke. I checked the FMS to ensure I had all the altitude restrs. I did not see the restr at korry or for that matter, initially, the fix korry itself. Korry is shown in an exploded diagram on the top left of the page -- that diagram does not show an altitude restr. The verbiage on the SID and used by ATC is/was 'descend via the korry 1.' ATC did not issue a limit altitude in the clearance, nor is a final altitude stated in the arrival verbiage. I opened the arrival to check for altitudes downstream of brand and having found korry in the exploded view did not notice the restr at korry. Before initiating the arrival, I again checked to ensure I had all the altitude restrs. I had 5 and still did not notice the altitude at korry. I queried the first officer 'I think I've got all the altitudes?' (don't recall response.) as we were descending toward brand, ATC issued a speed restr at korry. They did not mention the altitude. Shortly thereafter, they changed landing runways. This distraction us during the time I would have ben refolding the arrival to look at the terminal stages and I was occupied ensuring I would make the 'assigned' speed at korry' which was not in the initial descent planning. When the FMS reprogramming was done and my speed restr made, I unfolded the chart, saw the restr and made an immediate descent to 10000 ft. I was several mi late -- no comment from ATC. Recommend: 1) an altitude clearance limit be issued along with the arrival. 2) the altitude limit be included in the arrival description. 3) the crossing restr be shown to the exploded portion of the commercial chart arrival.

Narrative: KORRY 1 ARR TO LGA. THIS IS A NEW ARR TO LGA WHICH I HAD NEVER FLOWN. OUR COMMERCIAL CHART PAGE IS A FOLD-OUT, THE R PORTION OF WHICH I HAD FOLDED UNDER SO IT WOULD FIT ON MY MAP CLIP ON THE YOKE. I CHKED THE FMS TO ENSURE I HAD ALL THE ALT RESTRS. BEFORE INITIATING THE ARR, I AGAIN CHKED TO ENSURE I HAD ALL THE ALT RESTRS. I HAD 5 AND STILL DID NOT NOTICE THE ALT AT KORRY. I QUERIED THE FO 'I THINK I'VE GOT ALL THE ALTS?' (DON'T RECALL RESPONSE.) AS WE WERE DSNDING TOWARD BRAND, ATC ISSUED A SPD RESTR AT KORRY. THEY DID NOT MENTION THE ALT. SHORTLY THEREAFTER, THEY CHANGED LNDG RWYS. THIS DISTR US DURING THE TIME I WOULD HAVE BEN REFOLDING THE ARR TO LOOK AT THE TERMINAL STAGES AND I WAS OCCUPIED ENSURING I WOULD MAKE THE 'ASSIGNED' SPD AT KORRY' WHICH WAS NOT IN THE INITIAL DSCNT PLANNING. WHEN THE FMS REPROGRAMMING WAS DONE AND MY SPD RESTR MADE, I UNFOLDED THE CHART, SAW THE RESTR AND MADE AN IMMEDIATE DSCNT TO 10000 FT. I WAS SEVERAL MI LATE -- NO COMMENT FROM ATC. RECOMMEND: 1) AN ALT CLRNC LIMIT BE ISSUED ALONG WITH THE ARR. 2) THE ALT LIMIT BE INCLUDED IN THE ARR DESCRIPTION. 3) THE XING RESTR BE SHOWN TO THE EXPLODED PORTION OF THE COMMERCIAL CHART ARR.
