Narrative:

Aircraft was cleared for tiptoe visual approach to runway 28L. I read back clearance. Captain (PF) thought he was cleared for visual and began turn directly toward runway. I told him 'no, you were cleared for tiptoe visual.' he continued inbound after a second reminder. I then gave him a heading to comply with clearance. He continued directly to airport. He was high and close to traffic he was supposed to follow. Eventually we were told to go around. I believe that company needs to revisit CRM with this individual.

Narrative: ACFT WAS CLRED FOR TIPTOE VISUAL APCH TO RWY 28L. I READ BACK CLRNC. CAPT (PF) THOUGHT HE WAS CLRED FOR VISUAL AND BEGAN TURN DIRECTLY TOWARD RWY. I TOLD HIM 'NO, YOU WERE CLRED FOR TIPTOE VISUAL.' HE CONTINUED INBOUND AFTER A SECOND REMINDER. I THEN GAVE HIM A HDG TO COMPLY WITH CLRNC. HE CONTINUED DIRECTLY TO ARPT. HE WAS HIGH AND CLOSE TO TFC HE WAS SUPPOSED TO FOLLOW. EVENTUALLY WE WERE TOLD TO GO AROUND. I BELIEVE THAT COMPANY NEEDS TO REVISIT CRM WITH THIS INDIVIDUAL.
