Narrative:
On descent into tokyo narita airport (nrt), we were given clearance to 15000 ft. Captain was flying. First officer was trying to load a new waypoint into the FMS. Aircraft leveled at 15000 ft. First officer was having a little bit of difficulty finding new waypoint. I was the relief first officer and was helping the first officer set up the holding pattern. After getting the new waypoint, the new hold, and the new arrival put in the FMS, the first officer looked up and noticed that we were now at 10500 ft in a gradual descent to 10000 ft. First officer said 'why are we going to ten?' captain said we were cleared to ten with the holding instructions. First officer said he never heard that. I said I never heard that. Relief captain said he never heart that. About this time tokyo control asked our altitude. Said quickly 'you are cleared to ten (as a favor to us). The captain who misheard clearance was not wearing headset per company procedures and did not follow altitude procedures of pointing to altitude and having other pilot confirm. Not good head work after a 13 hour flight.

Title: ALT EXCURSION BY THE PIC OF AN MD11 IN HOLDING PATTERN 50 NM N OF NARITA, RJAA, FO.
Narrative: ON DSCNT INTO TOKYO NARITA ARPT (NRT), WE WERE GIVEN CLRNC TO 15000 FT. CAPT WAS FLYING. FO WAS TRYING TO LOAD A NEW WAYPOINT INTO THE FMS. ACFT LEVELED AT 15000 FT. FO WAS HAVING A LITTLE BIT OF DIFFICULTY FINDING NEW WAYPOINT. I WAS THE RELIEF FO AND WAS HELPING THE FO SET UP THE HOLDING PATTERN. AFTER GETTING THE NEW WAYPOINT, THE NEW HOLD, AND THE NEW ARR PUT IN THE FMS, THE FO LOOKED UP AND NOTICED THAT WE WERE NOW AT 10500 FT IN A GRADUAL DSCNT TO 10000 FT. FO SAID 'WHY ARE WE GOING TO TEN?' CAPT SAID WE WERE CLRED TO TEN WITH THE HOLDING INSTRUCTIONS. FO SAID HE NEVER HEARD THAT. I SAID I NEVER HEARD THAT. RELIEF CAPT SAID HE NEVER HEART THAT. ABOUT THIS TIME TOKYO CTL ASKED OUR ALT. SAID QUICKLY 'YOU ARE CLRED TO TEN (AS A FAVOR TO US).
