Narrative:
I have experienced 3 separate events in the past 36 months involving the failure of 1 brake disc while operating an F100 aircraft following a write-up by the flight crew of an inoperative brake temperature indicator. It appears that, after maintenance has changed a tire and wheel, the hydraulic line is not connected so as to allow hydraulic pressure to that single brake system. Subsequently, the cockpit indicates no brake temperature on the system, a write-up occurs, and is signed off as a brake temperature indicator failure and the problem goes undetected.

Title: A FOKKER CAPT RPTS 3 BRAKE FAILURES CAUSED BY MAINT NOT CONNECTING THE HYD PRESSURE LINE TO THE BRAKE ASSEMBLY. AFTER CHANGING A TIRE AND WHEEL.
Narrative: I HAVE EXPERIENCED 3 SEPARATE EVENTS IN THE PAST 36 MONTHS INVOLVING THE FAILURE OF 1 BRAKE DISC WHILE OPERATING AN F100 ACFT FOLLOWING A WRITE-UP BY THE FLC OF AN INOP BRAKE TEMP INDICATOR. IT APPEARS THAT, AFTER MAINT HAS CHANGED A TIRE AND WHEEL, THE HYD LINE IS NOT CONNECTED SO AS TO ALLOW HYD PRESSURE TO THAT SINGLE BRAKE SYS. SUBSEQUENTLY, THE COCKPIT INDICATES NO BRAKE TEMP ON THE SYS, A WRITE-UP OCCURS, AND IS SIGNED OFF AS A BRAKE TEMP INDICATOR FAILURE AND THE PROB GOES UNDETECTED.
