Narrative:
Landed with ill passenger, taxied off active runway after expedited short approach. Continued taxi, informed by ground controller I should have gotten clearance first. Was in a hurry due to medical problem on board, assumed tower so informed ground. On departure, got close to hold short line and wingtip was over the line. Moved to a second entrance well behind the hold short line. Lesson: always get an explicit taxi clearance. Don't assume or presuppose anything! Supplemental information from acn 549063: met by police, accompanied patient to FBO. Sat down for 1 hour, drank water. Felt better, departed uneventfully and completed mission.

Title: A C172 PLT, AFTER LNDG AT DXR, CONTINUED TO TAXI WITHOUT FIRST RECEIVING TAXI CLRNC.
Narrative: LANDED WITH ILL PAX, TAXIED OFF ACTIVE RWY AFTER EXPEDITED SHORT APCH. CONTINUED TAXI, INFORMED BY GND CTLR I SHOULD HAVE GOTTEN CLRNC FIRST. WAS IN A HURRY DUE TO MEDICAL PROB ON BOARD, ASSUMED TWR SO INFORMED GND. ON DEP, GOT CLOSE TO HOLD SHORT LINE AND WINGTIP WAS OVER THE LINE. MOVED TO A SECOND ENTRANCE WELL BEHIND THE HOLD SHORT LINE. LESSON: ALWAYS GET AN EXPLICIT TAXI CLRNC. DON'T ASSUME OR PRESUPPOSE ANYTHING!
