Narrative:
On commercial high altitude chart, on side 8, a line that connects deer park (dpk) to santt intersection, looks like it touches the VOR indicator. You think that la guardia is the VOR for J95 and the outbound 320 degree radial. The correct VOR is deer park (dpk). Assigned to intercept J95 from 340 degree heading. Instead intercepted lga 320 degree radial. Supplemental information from acn 553186: last leg of a 3-DAY trip. Filed to fly J95 after ewr SID. Clutter on hi 7/8 and hurried departure from gate contributed to both pilots setting up lga 320 degree radial instead of dpk 320 degree radial. This put us south of course. We were instructed by ATC to fly a heading then resume own navigation. Flag out of dpk on chart also added to problem. I have flown this SID before without incident.

WE WERE INSTRUCTED BY ATC TO FLY A HDG THEN RESUME OWN NAV. FLAG OUT OF DPK ON CHART ALSO ADDED TO PROB. I HAVE FLOWN THIS SID BEFORE WITHOUT INCIDENT.
