Narrative:
During preflight of my oxygen system, I found the oxygen hose disconnected. I plugged it in and the mask check worked ok. I noticed in the logbook where maintenance had replaced the oxygen mask. I figured that it had been installed improperly. The aircraft had flown about 3 times since then and I didn't write it up because I didn't want to cause those 3 pilots grief on any follow-up. I just saw a message on an oxygen hose that was disconnected. I called pilot union safety and they suggested I submit a report.

Title: MD80 CAPT FOUND HIS OXYGEN MASK NOT CONNECTED TO THE OXYGEN SUPPLY. 3 OTHER CAPT'S DID NOT CHK THEIR OXYGEN MASK.
Narrative: DURING PREFLT OF MY OXYGEN SYS, I FOUND THE OXYGEN HOSE DISCONNECTED. I PLUGGED IT IN AND THE MASK CHK WORKED OK. I NOTICED IN THE LOGBOOK WHERE MAINT HAD REPLACED THE OXYGEN MASK. I FIGURED THAT IT HAD BEEN INSTALLED IMPROPERLY. THE ACFT HAD FLOWN ABOUT 3 TIMES SINCE THEN AND I DIDN'T WRITE IT UP BECAUSE I DIDN'T WANT TO CAUSE THOSE 3 PLTS GRIEF ON ANY FOLLOW-UP. I JUST SAW A MESSAGE ON AN OXYGEN HOSE THAT WAS DISCONNECTED. I CALLED PLT UNION SAFETY AND THEY SUGGESTED I SUBMIT A RPT.
