Narrative:
ATC facility, dfw tower. Location: on the dam visual approach. On final to runway 18R at dfw, I called for flaps 5 degrees at 210 KIAS. Flaps froze in position at less than 1 degree on the flap indicator and 'leading edge flaps disagreement' EICAS message appeared on screen. We advised tower that we would have to discontinue the approach for a flap problem, emergency was declared and trucks were rolled. We completed emergency checklist and complied with all other procedures and landed with flaps 20 degrees. Approach and landing were uneventful. This was a third time repeat write-up in the log and the airplane was taken OTS by maintenance.
