Narrative:
Instrument panel was left unsecured after troubleshooting and repair of captain's pitot static system. As a result, the instrument panel fell out on the takeoff roll. Several items had an impact on my performance. Sleep deprivation/shift schedule. Distractionsecond officerther responsibilities, supervising other projects and crew, etc. Internal pressure to satisfy customer.

Title: A B737-400 ON TKOF ROLL HAD THE CAPT'S INST PANEL SLIDE OUT, CAUSED BY NOT BEING SECURED AFTER MAINT.
Narrative: INST PANEL WAS LEFT UNSECURED AFTER TROUBLESHOOTING AND REPAIR OF CAPT'S PITOT STATIC SYS. AS A RESULT, THE INST PANEL FELL OUT ON THE TKOF ROLL.
