Narrative:
I complied with the so cabin inspection in accordance with the company flight manual fire extinguishers check: 1) 3 mounted forward of cargo net or located at rcb. 2) on aircraft with rcb, fire extinguishers are connected to common manifold. 3) check condition of hoses. 4) ensure pressure indicator is in the green band and safety wire unbroken. 4) ensure fire extinguisher indicators match hose on non-rcb aircraft. Aircraft is an rcb aircraft and hoses were checked, safety wire checked and pressure was in the green. However, fire extinguisher inspection was expired. Recommend: placing dates for fire extinguishers inspection in the aircraft release document so crews have a common place to check dates for valid inspection. Supplemental information from acn 563546: I am a first officer on the DC10 for a cargo carrier. I received a company e-mail from the safety department informing me that on my last flight the DC10 we had flown had a hazmat fire extinguisher with an expired inspection date. It is the obligation of the so to inspect and confirm the extinguisher inspection dates. It is not company procedure for either the first officer or captain to inspect the extinguishers during preflight, or at any other time.
Original NASA ASRS Text
Title: A LOW TIME SO FAILS TO DETECT THE FACT THAT THE CABIN FIRE EXTINGUISHERS IN THE CARGO DC10 WERE OVERDUE FOR INSPECTION AT ZZZ, US.
Narrative: I COMPLIED WITH THE SO CABIN INSPECTION IN ACCORDANCE WITH THE COMPANY FLT MANUAL FIRE EXTINGUISHERS CHK: 1) 3 MOUNTED FORWARD OF CARGO NET OR LOCATED AT RCB. 2) ON ACFT WITH RCB, FIRE EXTINGUISHERS ARE CONNECTED TO COMMON MANIFOLD. 3) CHK CONDITION OF HOSES. 4) ENSURE PRESSURE INDICATOR IS IN THE GREEN BAND AND SAFETY WIRE UNBROKEN. 4) ENSURE FIRE EXTINGUISHER INDICATORS MATCH HOSE ON NON-RCB ACFT. ACFT IS AN RCB ACFT AND HOSES WERE CHKED, SAFETY WIRE CHKED AND PRESSURE WAS IN THE GREEN. HOWEVER, FIRE EXTINGUISHER INSPECTION WAS EXPIRED. RECOMMEND: PLACING DATES FOR FIRE EXTINGUISHERS INSPECTION IN THE ACFT RELEASE DOCUMENT SO CREWS HAVE A COMMON PLACE TO CHK DATES FOR VALID INSPECTION. SUPPLEMENTAL INFO FROM ACN 563546: I AM A FO ON THE DC10 FOR A CARGO CARRIER. I RECEIVED A COMPANY E-MAIL FROM THE SAFETY DEPT INFORMING ME THAT ON MY LAST FLT THE DC10 WE HAD FLOWN HAD A HAZMAT FIRE EXTINGUISHER WITH AN EXPIRED INSPECTION DATE. IT IS THE OBLIGATION OF THE SO TO INSPECT AND CONFIRM THE EXTINGUISHER INSPECTION DATES. IT IS NOT COMPANY PROC FOR EITHER THE FO OR CAPT TO INSPECT THE EXTINGUISHERS DURING PREFLT, OR AT ANY OTHER TIME.
