Narrative:
Hazmat specialist in syracuse, ny, loaded 5KG of dry ice in forward belly compartment of A310. This is not allowed. Captain missed it on the notification form and signed it. Once airborne, was SELCAL'ed by flight control and advised of situation. First officer went on oxygen as precaution as he was the PF. Flight diverted to cle and had the dry ice removed from the non allowable area. Hazmat form is generic to all aircraft types and I simply missed the small quantity which was listed. The form needs to be redesigned so that no prohibited entries can be made on it. Also, I need to be more careful to examine any form which I put my signature on. Supplemental information from acn 565452: a hazmat form specific to the A300/310 would help prevent this problem from occurring again.

Title: A310 CREW HAD DRY ICE LOADED IN THE FORWARD PIT IN NON COMPLIANCE WITH DANGEROUS GOODS LOADING REQUIREMENTS.
Narrative: HAZMAT SPECIALIST IN SYRACUSE, NY, LOADED 5KG OF DRY ICE IN FORWARD BELLY COMPARTMENT OF A310. THIS IS NOT ALLOWED. CAPT MISSED IT ON THE NOTIFICATION FORM AND SIGNED IT.
