Narrative:
After deicing at ZZZ with type 1 and type 4 fluid, I failed to get MEL 27-99 before takeoff. I didn't realize the error until we were about over pittsburgh and flight control got out the flight handbook bulletin. By that time, the only thing to do was put the fact that we had deiced with type 4 fluid in the logbook. Then we sent a commercial radio message to dispatch asking them to forward it to maintenance. I was aware of the 270 KT restr, and we did adhere to that, but didn't remember the requirement to get the MEL. Things were pretty hectic. First, it took over 20 mins just to push back from the gate, because the tug was fishtailing on the slush. Then, ground asked us if we would be blocking a taxiway during deicing and we said no on the advice of the pushback crew. Finally, the tug could get no more traction, so he had to unhook early and we started deicing though we were partially blocking the taxiway. This doesn't have anything to do with not getting the MEL, but it did raise the stress level.

Title: A B737-700 CREW LOGGED DEICING AT PREVIOUS STATION, BUT FAILED TO GET MEL DEFERRAL ON REQUIRED INSPECTION AND CLEANING.
Narrative: AFTER DEICING AT ZZZ WITH TYPE 1 AND TYPE 4 FLUID, I FAILED TO GET MEL 27-99 BEFORE TKOF. I DIDN'T REALIZE THE ERROR UNTIL WE WERE ABOUT OVER PITTSBURGH AND FLT CTL GOT OUT THE FLT HANDBOOK BULLETIN. BY THAT TIME, THE ONLY THING TO DO WAS PUT THE FACT THAT WE HAD DEICED WITH TYPE 4 FLUID IN THE LOGBOOK. THEN WE SENT A COMMERCIAL RADIO MESSAGE TO DISPATCH ASKING THEM TO FORWARD IT TO MAINT.
