Narrative:
Loss of main landing gear on takeoff. Departed ZZZ on runway 7L around XA30L. Advised by tower that a wheel had fallen off and was rolling down the runway. Antiskid EICAS illuminated and accomplished checklist. Went into holding sxc to burn off fuel. Decided to do a low fly-by during remaining daylight hours with lax tower. They advised wheel/tire missing on the right rear outboard position and inboard position looked flat. Back to holding to burn off fuel down to 20.0 then would start approach. During the 1 1/2 hour hold, we ran all checklists to include the antiskid EICAS. Gear up or partial gear landing, approach/descent, and final descent checklist. Coordinated with dispatch, maintenance instructors, ATC passenger, and flight attendants. 30 min prior to approach, we prepared passenger for evacuate/evacuation. Captain flew standard ILS approach to runway 7L. Gave passenger 'brace' call at 100 ft. Touchdown appeared normal with captain applying aileron to keep weight off of right main gear. No major directional control problems -- rolled to end of runway. Gave passenger 'remain seated' call. Taxied clear after tower reported no fire/smoke or anything abnormal. Maintenance then towed aircraft to maintenance area after cleared by rescue/response vehicles. Lessons learned -- we forgot to open the cockpit security bar after briefing it. This could have made it difficult to get us out if unconscious.

Title: A B767 ON TKOF CLB DECLARED AN EMER AND DIVERTED DUE TO TWR RPT OF THE LOSS OF THE R MAIN GEAR OUTBOARD WHEEL AND TIRE ASSEMBLY.
Narrative: LOSS OF MAIN LNDG GEAR ON TKOF. DEPARTED ZZZ ON RWY 7L AROUND XA30L. ADVISED BY TWR THAT A WHEEL HAD FALLEN OFF AND WAS ROLLING DOWN THE RWY. ANTISKID EICAS ILLUMINATED AND ACCOMPLISHED CHKLIST. WENT INTO HOLDING SXC TO BURN OFF FUEL. DECIDED TO DO A LOW FLY-BY DURING REMAINING DAYLIGHT HRS WITH LAX TWR. THEY ADVISED WHEEL/TIRE MISSING ON THE R REAR OUTBOARD POS AND INBOARD POS LOOKED FLAT. BACK TO HOLDING TO BURN OFF FUEL DOWN TO 20.0 THEN WOULD START APCH. DURING THE 1 1/2 HR HOLD, WE RAN ALL CHKLISTS TO INCLUDE THE ANTISKID EICAS. GEAR UP OR PARTIAL GEAR LNDG, APCH/DSCNT, AND FINAL DSCNT CHKLIST. COORDINATED WITH DISPATCH, MAINT INSTRUCTORS, ATC PAX, AND FLT ATTENDANTS. 30 MIN PRIOR TO APCH, WE PREPARED PAX FOR EVAC. CAPT FLEW STANDARD ILS APCH TO RWY 7L. GAVE PAX 'BRACE' CALL AT 100 FT. TOUCHDOWN APPEARED NORMAL WITH CAPT APPLYING AILERON TO KEEP WT OFF OF R MAIN GEAR. NO MAJOR DIRECTIONAL CTL PROBS -- ROLLED TO END OF RWY. GAVE PAX 'REMAIN SEATED' CALL. TAXIED CLR AFTER TWR RPTED NO FIRE/SMOKE OR ANYTHING ABNORMAL. MAINT THEN TOWED ACFT TO MAINT AREA AFTER CLRED BY RESCUE/RESPONSE VEHICLES. LESSONS LEARNED -- WE FORGOT TO OPEN THE COCKPIT SECURITY BAR AFTER BRIEFING IT. THIS COULD HAVE MADE IT DIFFICULT TO GET US OUT IF UNCONSCIOUS.
