Narrative:

After deicing and taxi to runway 10C, the left generator dropped off-line. Complied with MEL and the left bus was pwred by APU. At approximately V1, the right generator dropped off-line. Climbed to 5000 ft, leveled off and turned downwind. Per abnormal procedures checklist, the right generator would not reset and the right bus load picked up by the APU. Executed an uneventful landing and returned to gate. Problem: faulty electrical system. Problem: not informing flight dispatch of situation with left generator on the ground (prior to takeoff). Problem: improper write-up in logbook (postflt). Corrective actions: 1) inform dispatch of all sits prior to takeoff. 2) insure write-up is actually the occurrence that happened in the proper time line.

Title: AFTER LOSING THE L GENERATOR AND SUPPLEMENTING REQUIREMENTS WITH THE APU, AN MD88 FLT CREW LOSES ITS SECOND GENERATOR AT V1 AND AFTER TKOF RETURNS/LANDS AT PIT, PA.

Narrative: AFTER DEICING AND TAXI TO RWY 10C, THE L GENERATOR DROPPED OFF-LINE. COMPLIED WITH MEL AND THE L BUS WAS PWRED BY APU. AT APPROX V1, THE R GENERATOR DROPPED OFF-LINE. CLBED TO 5000 FT, LEVELED OFF AND TURNED DOWNWIND. PER ABNORMAL PROCS CHKLIST, THE R GENERATOR WOULD NOT RESET AND THE R BUS LOAD PICKED UP BY THE APU. EXECUTED AN UNEVENTFUL LNDG AND RETURNED TO GATE. PROB: FAULTY ELECTRICAL SYS. PROB: NOT INFORMING FLT DISPATCH OF SIT WITH L GENERATOR ON THE GND (PRIOR TO TKOF). PROB: IMPROPER WRITE-UP IN LOGBOOK (POSTFLT). CORRECTIVE ACTIONS: 1) INFORM DISPATCH OF ALL SITS PRIOR TO TKOF. 2) INSURE WRITE-UP IS ACTUALLY THE OCCURRENCE THAT HAPPENED IN THE PROPER TIME LINE.
