Narrative:
While departing msy, the cockpit door swung open during rotation. The #1 flight attendant tried repeatedly the close it, but she had to hold it closed until our flight path stabilized. Upon closer inspection, we realized that during the before takeoff checklist item 'cockpit door,' we had failed to catch the fact that the power switch was off. In the future, 'cockpit door secure' will also include 'power switch on.'

Title: UNSECURED COCKPIT FORTRESS DOOR OPENED DURING TAKEOFF ROTATION.
Narrative: WHILE DEPARTING MSY, THE COCKPIT DOOR SWUNG OPEN DURING ROTATION. THE #1 FLT ATTENDANT TRIED REPEATEDLY THE CLOSE IT, BUT SHE HAD TO HOLD IT CLOSED UNTIL OUR FLT PATH STABILIZED. UPON CLOSER INSPECTION, WE REALIZED THAT DURING THE BEFORE TAKEOFF CHKLIST ITEM 'COCKPIT DOOR,' WE HAD FAILED TO CATCH THE FACT THAT THE POWER SWITCH WAS OFF. IN THE FUTURE, 'COCKPIT DOOR SECURE' WILL ALSO INCLUDE 'POWER SWITCH ON.'
