Narrative:
I climbed DH8 to 12000 ft, then reissued 13000 ft, 10 mi in trail I climbed to 13000 ft. I thought the DH8 was climbing to 12000 ft. I tried to turn the B737, but I xferred communication. I also issued traffic to the B737 about the DH8, who (B737) had in sight.

Title: PHL APCH CTLR CLRED A B733 TO 13000 FT, BUT FORGOT HE HAD DH8 ON THE SAME COURSE AND CLRED TO THE SAME ALT. LOSS OF SEPARATION OCCURRED.
Narrative: I CLBED DH8 TO 12000 FT, THEN REISSUED 13000 FT, 10 MI IN TRAIL I CLBED TO 13000 FT. I THOUGHT THE DH8 WAS CLBING TO 12000 FT. I TRIED TO TURN THE B737, BUT I XFERRED COM. I ALSO ISSUED TFC TO THE B737 ABOUT THE DH8, WHO (B737) HAD IN SIGHT.
