Narrative:
Slide failed to deploy. I was told this later. I installed the slide as per card XXXX. To the best of my knowledge it was done per the card. I inspected per the card with no known discreprencies. Callback conversation with reporter revealed the following information: the reporter stated the aircraft emergency evacuation occurred to an md-80 while parked on a gate when smoke filled the passenger cabin. The reporter said the rear tailcone exit could not be used because after the tailcone dropped off the escape slide did not deploy. The reporter stated he was one of two technicians that installed the slide pack and the job card was followed and the installation was inspected per the job card. The reporter said during investigation it was revealed the cause of the slide deployment failure was unknown. The reporter stated a lanyard was found out of position on the slide pack and it could not be determined if this was a shop error or happened in transit or during installation.

Title: AN MD SUPER 80 DURING AN EMER EVACUATION THE TAILCONE ESCAPE SLIDE FAILED TO DEPLOY. TAILCONE DROPPED OFF.
Narrative: SLIDE FAILED TO DEPLOY. I WAS TOLD THIS LATER. I INSTALLED THE SLIDE AS PER CARD XXXX. TO THE BEST OF MY KNOWLEDGE IT WAS DONE PER THE CARD. I INSPECTED PER THE CARD WITH NO KNOWN DISCREPRENCIES. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: THE RPTR STATED THE ACFT EMER EVACUATION OCCURRED TO AN MD-80 WHILE PARKED ON A GATE WHEN SMOKE FILLED THE PAX CABIN. THE RPTR SAID THE REAR TAILCONE EXIT COULD NOT BE USED BECAUSE AFTER THE TAILCONE DROPPED OFF THE ESCAPE SLIDE DID NOT DEPLOY. THE RPTR STATED HE WAS ONE OF TWO TECHNICIANS THAT INSTALLED THE SLIDE PACK AND THE JOB CARD WAS FOLLOWED AND THE INSTALLATION WAS INSPECTED PER THE JOB CARD. THE RPTR SAID DURING INVESTIGATION IT WAS REVEALED THE CAUSE OF THE SLIDE DEPLOYMENT FAILURE WAS UNKNOWN.
