Narrative:
Captain flying pilot. Just after lift off 200-300 ft AGL right rudder full deflection and rudder system 1 & 2 fail message on EICAS. Captain flew, I did radios talked with ZZZ maintenance and flight attendant. Declared emergency with approach and got vectors back to ILS runway 33. We both had to apply left rudder inputs together to maintain directional control of aircraft. Captain handled ailerons and I did thrust levers on approach and braced passenger. Upon landing captain was able to use nose, wheel steering and once aircraft was stopped, took over thrust levers, taxied to gate. Everyone deplaned safely and uneventfully. Callback conversation with reporter revealed the following information: when on the ground, maintenance found the input rod to the rudder power unit was disconnected. The reporter said maintenance did not advise whether the rod end of the rudder input rod had broken or the fastener connecting the input rod end to the rudder power unit had failed.

Title: AN EMBRAER 135, AFTER LIFTOFF AT 300 FT HAD RUDDER DEFLECTION WITH EICAS 'RUDDER 1 & 2 FAIL' WARNING. DECLARED EMER AND DIVERTED.
Narrative: CAPT FLYING PLT. JUST AFTER LIFT OFF 200-300 FT AGL R RUDDER FULL DEFLECTION AND RUDDER SYSTEM 1 & 2 FAIL MESSAGE ON EICAS. CAPT FLEW, I DID RADIOS TALKED WITH ZZZ MAINT AND FA. DECLARED EMER WITH APCH AND GOT VECTORS BACK TO ILS RWY 33. WE BOTH HAD TO APPLY L RUDDER INPUTS TOGETHER TO MAINTAIN DIRECTIONAL CONTROL OF ACFT. CAPT HANDLED AILERONS AND I DID THRUST LEVERS ON APCH AND BRACED PAX. UPON LNDG CAPT WAS ABLE TO USE NOSE, WHEEL STEERING AND ONCE ACFT WAS STOPPED, TOOK OVER THRUST LEVERS, TAXIED TO GATE. EVERYONE DEPLANED SAFELY AND UNEVENTFULLY. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: WHEN ON THE GROUND, MAINT FOUND THE INPUT ROD TO THE RUDDER POWER UNIT WAS DISCONNECTED. THE RPTR SAID MAINT DID NOT ADVISE WHETHER THE ROD END OF THE RUDDER INPUT ROD HAD BROKEN OR THE FASTENER CONNECTING THE INPUT ROD END TO THE RUDDER POWER UNIT HAD FAILED.
