Narrative:
Passenger boarded aircraft with dog draped over the back of her shoulder. Flight attendant #4 approached passenger stating that dog must be in carrier. Passenger said she would once she got settled in seat. This was during boarding process. During safety demonstration, flight attendant #2 noticed that dog was still out of carrier. Passenger said she would comply. After takeoff, during beverage cart service, flight attendants #4 and #2 noticed that passenger still had dog's head fully outside of carrier. Flight attendant #4 then informed passenger that dog must be completely inside carrier at all times. Passenger made attempt to comply but did not follow through. On 4TH approach, after service, flight attendant #4 checked on passenger compliance. Passenger still had dog's head fully outside of carrier. Flight attendant #4 stated that compliance was needed and also issued an in-flight disturbance report. Obtained passenger's address, etc. Also obtained witness name/phone/address.

Title: AN MD82 RPTED THAT A PAX TRANSPORTED HER DOG IN A MANNER NOT IN COMPLIANCE WITH FARS.
Narrative: PAX BOARDED ACFT WITH DOG DRAPED OVER THE BACK OF HER SHOULDER. FLT ATTENDANT #4 APCHED PAX STATING THAT DOG MUST BE IN CARRIER. PAX SAID SHE WOULD ONCE SHE GOT SETTLED IN SEAT. THIS WAS DURING BOARDING PROCESS. DURING SAFETY DEMONSTRATION, FLT ATTENDANT #2 NOTICED THAT DOG WAS STILL OUT OF CARRIER. PAX SAID SHE WOULD COMPLY. AFTER TKOF, DURING BEVERAGE CART SVC, FLT ATTENDANTS #4 AND #2 NOTICED THAT PAX STILL HAD DOG'S HEAD FULLY OUTSIDE OF CARRIER. FLT ATTENDANT #4 THEN INFORMED PAX THAT DOG MUST BE COMPLETELY INSIDE CARRIER AT ALL TIMES. PAX MADE ATTEMPT TO COMPLY BUT DID NOT FOLLOW THROUGH. ON 4TH APCH, AFTER SVC, FLT ATTENDANT #4 CHKED ON PAX COMPLIANCE. PAX STILL HAD DOG'S HEAD FULLY OUTSIDE OF CARRIER. FLT ATTENDANT #4 STATED THAT COMPLIANCE WAS NEEDED AND ALSO ISSUED AN INFLT DISTURBANCE RPT. OBTAINED PAX'S ADDRESS, ETC. ALSO OBTAINED WITNESS NAME/PHONE/ADDRESS.
