Narrative:

Event is a forced landing, in field. No injuries to crew, aircraft or property. Cause: loose fuel feed line, allowing air into system after maintenance. Initial cause: most likely a failure to secure fuel line securely, or to verify security. Engine leak checks performed on ground after maintenance, successful. Engine run up check normal. Corrective action: develop an independent check and balance system to assure connections are secure. Develop a securing system for 'B-nuts' safety wire provisions, lock feature. Identify a better system to verify that all fittings are actually secure since the ground pump check and ground run proved successful with no indication of potential failure. Pilot stated that takeoff performance was not normal. However, continued takeoff. Develop a better system to abort takeoff?

Title: BE36 EXPERIENCES ENG FAILURE DUE TO FUEL SYS LEAKING AIR FROM AN IMPROPERLY SECURED FUEL FITTING.
