Narrative:
Pressurization. Flight was dispatched to mia with the l-hand pack inoperative. Flight limited to maximum altitude of FL350. At FL350 approaching ursus intersection northbound, cabin altitude warning light illuminated with associated aural warning. Executed red box procedures for loss of cabin altitude, including deploying passenger oxygen. Declared an emergency with havana center and started emergency descent. Shortly after initiating descent, cabin altitude exceeded 10000 ft. Contacted ZMA for continued descent and direct routing to miami (destination). Descended to below 10000 ft immediately. Completed appropriate checklists and landed at destination with emergency vehicles standing by.

Narrative: PRESSURIZATION. FLT WAS DISPATCHED TO MIA WITH THE L-HAND PACK INOP. FLT LIMITED TO MAX ALT OF FL350. AT FL350 APCHING URSUS INTXN NBOUND, CABIN ALT WARNING LIGHT ILLUMINATED WITH ASSOCIATED AURAL WARNING. EXECUTED RED BOX PROCS FOR LOSS OF CABIN ALT, INCLUDING DEPLOYING PAX OXYGEN. DECLARED AN EMER WITH HAVANA CTR AND STARTED EMER DSCNT. SHORTLY AFTER INITIATING DSCNT, CABIN ALT EXCEEDED 10000 FT. CONTACTED ZMA FOR CONTINUED DSCNT AND DIRECT ROUTING TO MIAMI (DEST). DSNDED TO BELOW 10000 FT IMMEDIATELY. COMPLETED APPROPRIATE CHKLISTS AND LANDED AT DEST WITH EMER VEHICLES STANDING BY.
