Narrative:

On approach to ZZZ on STAR to ILS runway 25L, I began to slow per ATC. When we extended flaps from flaps 1 degree to flaps 5 degrees, we saw EICAS caution trailing edge flap disagree. We broke off the approach for delay vectors, captain ran checklist while I flew and operated radios. Checklist was very confusing, as B757, B767-200 and B767-300 procedures were intermingled. We both read it twice before finding correct action steps and deploying flaps to 20 degrees using alternate extension. We decided to declare an emergency, advised dispatch and maintenance control. Captain gave information to purser for cabin advisory, and briefly informed passenger of situation. Approach and landing were uneventful, and we taxied in requiring no assistance. Good CRM by captain in asking me to read checklist to confirm correct action. I highly recommend revising the checklists to be specific to the airplane (ie, B757, B767-200, B767-300), instead of the current confusing and misleading format.
