Narrative:
On sep/tue/04, I received a letter from aviation safety inspector from the certification management office. His letter states that the aft entry door slide was installed incorrectly. Records show I installed the slide on mar/wed/04. His letter states the discovery was made during routine maintenance, at an unknown time and location. The discrepancy is also unknown. Records from air traffic area chapters 25 and 52 indicate the slide may have been removed and reinstalled after other maintenance on dates after the initial installation.

Title: A B737-500 TECHNICIAN RPTS BEING SUBJECTED TO AN FAA INVESTIGATION ON IMPROPER AFT ENTRY DOOR ESCAPE SLIDE INSTALLATION. POSSIBLE WORK IN AREA AFTER INSTALLATION.
Narrative: ON SEP/TUE/04, I RECEIVED A LETTER FROM AVIATION SAFETY INSPECTOR FROM THE CERTIFICATION MGMNT OFFICE. HIS LETTER STATES THAT THE AFT ENTRY DOOR SLIDE WAS INSTALLED INCORRECTLY. RECORDS SHOW I INSTALLED THE SLIDE ON MAR/WED/04. HIS LETTER STATES THE DISCOVERY WAS MADE DURING ROUTINE MAINT, AT AN UNKNOWN TIME AND LOCATION. THE DISCREPANCY IS ALSO UNKNOWN.
