Narrative:
On shutdown checklist pitot tube heat may have been left on, or after deplaning, ramp personnel could have turned it on, or pitot cover may have been installed before pitot tube cooled. Pitot tube cover melted onto pitot tube. Late at night fatigue could have been an issue, although the first officer and I checked it 4 times. We could have missed it (most likely). Supplemental information from acn 636950: we completed a trip in aty with after landing checklists and shutdown/termination checklists. Retired to hotel. Morning crew found pitot cover to be melted onto pitot tube. Human factor: we may have left pitot switch on.

Narrative: ON SHUTDOWN CHKLIST PITOT TUBE HEAT MAY HAVE BEEN LEFT ON, OR AFTER DEPLANING, RAMP PERSONNEL COULD HAVE TURNED IT ON, OR PITOT COVER MAY HAVE BEEN INSTALLED BEFORE PITOT TUBE COOLED. PITOT TUBE COVER MELTED ONTO PITOT TUBE. LATE AT NIGHT FATIGUE COULD HAVE BEEN AN ISSUE, ALTHOUGH THE FO AND I CHKED IT 4 TIMES. WE COULD HAVE MISSED IT (MOST LIKELY).
