Narrative:

Aircraft system failure pressurization. Aircraft was dispatched with the right air conditioning pack inoperative (this was a previous write-up) and 'AFB cabin' trim modulating valve inoperative. Dispatched at FL240 cruise altitude due to single pack operation. During climb out, prior to leveling off at 5000 ft, different, the #2 'bleed trip' light illuminated. Complied with the QRH procedure stating to push the 'trip reset' button. The bleed reset, and the light extinguished. Climbing through 15000 ft, the 'bleed trip' light illuminated again. We were in moderate icing conditions at that time, per manual, we were unable to reset a second trip. Still in moderate icing conditions, returned to ZZZ and declared an emergency due to an overweight landing. Landed on runway 27L, flaps 30 degrees, autobrakes #2 at a landing weight of approximately 154000 pounds. Smooth landing, uneventful touchdown and rollout. Taxied to the gate.

Title: B737-800 CREW WAS DISPATCHED WITH A MEL'ED PACK INOP. THE OTHER PACK HAD A 'BLEED TRIP' LIGHT ILLUMINATE.

Narrative: ACFT SYS FAILURE PRESSURIZATION. ACFT WAS DISPATCHED WITH THE R AIR CONDITIONING PACK INOP (THIS WAS A PREVIOUS WRITE-UP) AND 'AFB CABIN' TRIM MODULATING VALVE INOP. DISPATCHED AT FL240 CRUISE ALT DUE TO SINGLE PACK OP. DURING CLBOUT, PRIOR TO LEVELING OFF AT 5000 FT, DIFFERENT, THE #2 'BLEED TRIP' LIGHT ILLUMINATED. COMPLIED WITH THE QRH PROC STATING TO PUSH THE 'TRIP RESET' BUTTON. THE BLEED RESET, AND THE LIGHT EXTINGUISHED. CLBING THROUGH 15000 FT, THE 'BLEED TRIP' LIGHT ILLUMINATED AGAIN. WE WERE IN MODERATE ICING CONDITIONS AT THAT TIME, PER MANUAL, WE WERE UNABLE TO RESET A SECOND TRIP. STILL IN MODERATE ICING CONDITIONS, RETURNED TO ZZZ AND DECLARED AN EMER DUE TO AN OVERWT LNDG. LANDED ON RWY 27L, FLAPS 30 DEGS, AUTOBRAKES #2 AT A LNDG WT OF APPROX 154000 LBS. SMOOTH LNDG, UNEVENTFUL TOUCHDOWN AND ROLLOUT. TAXIED TO THE GATE.
