Narrative:
I received a letter of investigation from my foreman. The letter of investigation content entailed issues involving aircraft X and the non deployment of aft entry door slide during a deployment test. It also states 'slide failed to deploy due to the fact that the quick disconnect was missing.' I remember; before installation of new pack onto door; I had to remove the old pack board from door (old chute had deployment test or blown at gate -- can't remember for sure). I removed the 2 bolts (hex head) from old pack board and used same bolts to install new pack -- assuming they were correct. Upon review of letter of investigation report and airbus maintenance manual 25-62-44; I realize these were not the correct bolts. Since they were the incorrect bolts; I didn't realize the pull handles needed to be attached to them; so the pull handles were never connected.

Title: AN A319 DURING AN AFT ENTRY DOOR SLIDE DEPLOYMENT TEST THE SLIDE FAILED TO DEPLOY DUE TO INCORRECT INSTALLATION OF SLIDE. PULL HANDLES WERE FOUND NOT CONNECTED.
Narrative: I RECEIVED A LETTER OF INVESTIGATION FROM MY FOREMAN. THE LETTER OF INVESTIGATION CONTENT ENTAILED ISSUES INVOLVING ACFT X AND THE NON DEPLOYMENT OF AFT ENTRY DOOR SLIDE DURING A DEPLOYMENT TEST. IT ALSO STATES 'SLIDE FAILED TO DEPLOY DUE TO THE FACT THAT THE QUICK DISCONNECT WAS MISSING.'
