Narrative:
Upon pushback and engine start with a full aircraft; the flight attendant called to say that a large passenger in seat xxa (who should have been required to buy 2 seats) was crushing the girl next to him. We told them to put the girl in a seat occupied by a deadheading pilot and bring the deadheading pilot up to the cockpit. During all the confusion; we were distraction enough to start taxi before the ground crew was completely clear and we taxied over the tow bar after rolling forward 4 ft. We returned to the gate. No one was injured and there was no damage to the aircraft as determined by maintenance. Everything happened 'just right' for this to have happened and caused us to miss the fact that we did not have 'salute and release.' we should have slowed down a little and never answered the cabin call during such a busy time.

Title: B757 FLT CREW HAS A DIFFICULT PAX DURING PUSHBACK AND IS DISTR. INADVERTENTLY FORGETS TO COMPLETE PUSHBACK CHKLIST AND RECEIVE 'SALUTE; RELEASED FROM GUIDANCE.' ACFT TAXIES ACROSS THE TOW BAR AND RETURNS TO THE GATE FOR INSPECTION.
Narrative: UPON PUSHBACK AND ENG START WITH A FULL ACFT; THE FLT ATTENDANT CALLED TO SAY THAT A LARGE PAX IN SEAT XXA (WHO SHOULD HAVE BEEN REQUIRED TO BUY 2 SEATS) WAS CRUSHING THE GIRL NEXT TO HIM.
