Narrative:
Aircraft had a loud squeal from the service door. Pressurization was deferred in automatic and manual mode per MEL. MEL requires both outflow valves to be secured open with opening tool. Maintenance resealed rubber door trim and lower service kick plate in accordance with 52-00-04-300-801-a; with a notation that the MEL item was restored. Aircraft left and was unable to pressurize aircraft in automatic or manual mode. Upon further investigation it was determined that the outflow opening tool was not removed from the outflow valves. MEL was not restored properly and an operations check of the system and the noise from the door was not performed.

Title: AN EMB145 WITH PRESSURIZATION INOP IN MANUAL AND AUTO DEFERRED AS INOP. CORRECTED PRESSURE LEAK AT SVC DOOR. DISPATCHED ACFT WITH OUTFLOW VALVE LOCKED OPEN.
Narrative: ACFT HAD A LOUD SQUEAL FROM THE SVC DOOR. PRESSURIZATION WAS DEFERRED IN AUTO AND MANUAL MODE PER MEL. MEL REQUIRES BOTH OUTFLOW VALVES TO BE SECURED OPEN WITH OPENING TOOL. MAINT RESEALED RUBBER DOOR TRIM AND LOWER SVC KICK PLATE IN ACCORDANCE WITH 52-00-04-300-801-A; WITH A NOTATION THAT THE MEL ITEM WAS RESTORED. ACFT LEFT AND WAS UNABLE TO PRESSURIZE ACFT IN AUTO OR MANUAL MODE. UPON FURTHER INVESTIGATION IT WAS DETERMINED THAT THE OUTFLOW OPENING TOOL WAS NOT REMOVED FROM THE OUTFLOW VALVES. MEL WAS NOT RESTORED PROPERLY AND AN OPS CHK OF THE SYS AND THE NOISE FROM THE DOOR WAS NOT PERFORMED.
