Narrative:

Started the day at XA20 for a 5 leg day and 14 hour 6 min duty day. The first 3 legs went without incident. After a 2 hour 42 min situation and a late arriving; we did a crew change and quick-turn from ZZZ2 to ZZZ3. Then ATC informed us of a flow time wheels up time of XM24. Our original departure time was XL00. We took off at XM20 and in cruise 20 mi southwest of abc; realized that we needed to start down for ZZZ3 from FL280. ATC informed us that we had missed our frequency change and after 4 frequency changes we got on with center to start our descent. We made our descent with no problem but we missed our frequency changes. We were both getting a little tired from over 12 hours on duty; and fatigue was setting in. We landed without incident. We pushed from the gate in ZZZ3 for our return flight back to ZZZ2 at XO25. This is 13 hours of duty and counting. We are with ZZZ2 approach being vectored for runway 36R the first officer briefs runway 36L instead and I do not catch it. He programs runway 36L in the FMS as well. All the radio instructions and readback calls are for runway 36R but we end up intercepting the runway 36L localizer. Approach never catches this either; the tower controller tells us to turn right to 030 degrees to intercept the runway 36R localizer because we intercepted the runway 36L localizer instead. There were no other aircraft in the area for a conflict; but we clearly made a glaring mistake. We landed on runway 36R without incident and blocked in at XQ10. 15 mins of postflt duty and our day was 15 hours 5 mins of duty time on our 4TH day of 5 days of flying. Fatigue was definitely in play here; along with us trying to stay alert; which we failed to do so.

WE TOOK OFF AT XM20 AND IN CRUISE 20 MI SW OF ABC; REALIZED THAT WE NEEDED TO START DOWN FOR ZZZ3 FROM FL280. ATC INFORMED US THAT WE HAD MISSED OUR FREQ CHANGE AND AFTER 4 FREQ CHANGES WE GOT ON WITH CENTER TO START OUR DSCNT. WE MADE OUR DSCNT WITH NO PROB BUT WE MISSED OUR FREQ CHANGES. WE WERE BOTH GETTING A LITTLE TIRED FROM OVER 12 HRS ON DUTY; AND FATIGUE WAS SETTING IN. WE LANDED WITHOUT INCIDENT. WE PUSHED FROM THE GATE IN ZZZ3 FOR OUR RETURN FLT BACK TO ZZZ2 AT XO25. THIS IS 13 HRS OF DUTY AND COUNTING. WE ARE WITH ZZZ2 APCH BEING VECTORED FOR RWY 36R THE FO BRIEFS RWY 36L INSTEAD AND I DO NOT CATCH IT. HE PROGRAMS RWY 36L IN THE FMS AS WELL. THERE WERE NO OTHER ACFT IN THE AREA FOR A CONFLICT; BUT WE CLRLY MADE A GLARING MISTAKE. WE LANDED ON RWY 36R WITHOUT INCIDENT AND BLOCKED IN AT XQ10. 15 MINS OF POSTFLT DUTY AND OUR DAY WAS 15 HRS 5 MINS OF DUTY TIME ON OUR 4TH DAY OF 5 DAYS OF FLYING. FATIGUE WAS DEFINITELY IN PLAY HERE; ALONG WITH US TRYING TO STAY ALERT; WHICH WE FAILED TO DO SO.
