Narrative:

Departure control asked us to change our beacon code upon check-in. At that point the first officer realized he had not pulled up the pre departure clearance. When we did the checklist; I thought the pre departure clearance by the transponder was the correct one and only looked at the code. Everything looked like it was as filed so I didn't look very close at the other writing on the paper. I will next time.

Title: AN MD80 CREW FAILED TO REQUEST A PDC PRIOR TO DEP AND TOOK OFF WITHOUT THE CORRECT XPONDER CODE.

Narrative: DEP CTL ASKED US TO CHANGE OUR BEACON CODE UPON CHK-IN. AT THAT POINT THE FO REALIZED HE HAD NOT PULLED UP THE PDC. WHEN WE DID THE CHKLIST; I THOUGHT THE PDC BY THE XPONDER WAS THE CORRECT ONE AND ONLY LOOKED AT THE CODE. EVERYTHING LOOKED LIKE IT WAS AS FILED SO I DIDN'T LOOK VERY CLOSE AT THE OTHER WRITING ON THE PAPER. I WILL NEXT TIME.
