Narrative:
An airworthiness release was not issued upon the completion of aco (approved change order) eyebrow window deletion -- left side. This aco was considered to be a major alteration. Supplemental information from acn 717851: I received a letter from quality control informing me that I performed an eyebrow window deletion modification but did not give the aircraft an airworthiness release. The aircraft was assigned a maintenance visit; which does not require an airworthiness release; but the modification was considered a major alteration; which does require an airworthiness release. It's not that I forgot; I followed the paperwork steps but they did not include a signoff to perform an airworthiness release. In my memory this is the first modification in a line overnight situation that was a major alteration. We were doing several a night so I guess no mechanic or quality control caught on until the B-700 fleet was done or close to it.
Original NASA ASRS Text
Title: A B737-700 APPROVED CHANGE ORDER TO REPLACE EYEBROW WINDOWS WITH PLUGS WAS ACCOMPLISHED IN ACCORDANCE WITH FAA FORM 337. TECHNICIAN FAILED TO SIGN AIRWORTHINESS RELEASE ON COMPLETION OF WORK.
Narrative: AN AIRWORTHINESS RELEASE WAS NOT ISSUED UPON THE COMPLETION OF ACO (APPROVED CHANGE ORDER) EYEBROW WINDOW DELETION -- L SIDE. THIS ACO WAS CONSIDERED TO BE A MAJOR ALTERATION. SUPPLEMENTAL INFO FROM ACN 717851: I RECEIVED A LETTER FROM QUALITY CTL INFORMING ME THAT I PERFORMED AN EYEBROW WINDOW DELETION MODIFICATION BUT DID NOT GIVE THE ACFT AN AIRWORTHINESS RELEASE. THE ACFT WAS ASSIGNED A MAINT VISIT; WHICH DOES NOT REQUIRE AN AIRWORTHINESS RELEASE; BUT THE MODIFICATION WAS CONSIDERED A MAJOR ALTERATION; WHICH DOES REQUIRE AN AIRWORTHINESS RELEASE. WE WERE DOING SEVERAL A NIGHT SO I GUESS NO MECH OR QUALITY CTL CAUGHT ON UNTIL THE B-700 FLEET WAS DONE OR CLOSE TO IT.
