Narrative:
Normal pushback and taxi. Upon rotation (captain flying) felt moderate airframe vibration. At power reduction; vibration dramatically decreased. It was determined the #2 engine was the source of the vibration and the engine severe damage qrc was accomplished and cabin preparation issued to the flight attendants. The engine was left at idle and a 15 degree flap approach was flown. Uneventful landing and the engine was shut down after the aircraft was stopped. Fire department inspected the aircraft and reported severe damage to #2 engine. Taxi back to gate and shutdown were normal. Supplemental information from acn 718251: as I rotated I saw something coming onto the runway.

Title: B737-500 RETURNS TO DEP ARPT FOLLOWING SEVERE DAMAGE TO R ENG ON TKOF.
Narrative: NORMAL PUSHBACK AND TAXI. UPON ROTATION (CAPT FLYING) FELT MODERATE AIRFRAME VIBRATION. AT PWR REDUCTION; VIBRATION DRAMATICALLY DECREASED. IT WAS DETERMINED THE #2 ENG WAS THE SOURCE OF THE VIBRATION AND THE ENG SEVERE DAMAGE QRC WAS ACCOMPLISHED AND CABIN PREPARATION ISSUED TO THE FLT ATTENDANTS. THE ENG WAS LEFT AT IDLE AND A 15 DEG FLAP APCH WAS FLOWN. UNEVENTFUL LNDG AND THE ENG WAS SHUT DOWN AFTER THE ACFT WAS STOPPED. FIRE DEPT INSPECTED THE ACFT AND RPTED SEVERE DAMAGE TO #2 ENG. TAXI BACK TO GATE AND SHUTDOWN WERE NORMAL. SUPPLEMENTAL INFO FROM ACN 718251: AS I ROTATED I SAW SOMETHING COMING ONTO THE RWY.
