Narrative:

The controllers at dulles tower (iad) are forced to provide visual separation between aircraft on approach to runways 19R/left and 12. A combination of instrument and/or visual approachs are being run simultaneously to these 3 runways. If visual separation can't be provided by the tower; potomac TRACON (pct) is required to stagger these approachs in order to ensure IFR separation to the threshold and protect for a missed approach. This procedure violates faao 7110.65 7-4-4 approachs to multiple runways. When using visual separation; approachs should only be run to 2 runways not 3 (given the runway confign at dulles).
