Narrative:
I was on vacation a week. I was told that my chief inspector; quit on jun/xa/07. Mr X was the mechanic who started the installation for the first 3 cylinders. Then approximately 3 weeks later; another mechanic was assigned to install the other 3 cylinders that arrived last; without knowing that the first 3 cylinders were not torqued properly. We want to take responsibility for our actions. We have hired another chief inspector. We have scheduled refresher training for our mechanics in reference to inspection requirements (rii).
