Narrative:
I was informed that one of my flts was released with MEL 28-X; but it did not have MEL 28-Y attached. Provisos of first MEL states that second MEL will also be placarded. I look at the MEL book for each MEL; but missed this particular provisos for adding the additional MEL. I will be more vigilant in reading mels. Callback conversation with reporter revealed the following information: reporter stated maintenance apparently did not see the requirement under the first MEL provisos to add an additional MEL and placard prior to aircraft release. Because this additional MEL was not input into their maintenance release data base; the aircraft flew many flights with multiple layers of groups; including mechanics; flight crews and dispatchers; missing the requirement for a second MEL procedure.

Title: AN MD80 ACFT DISPATCHER REPORTS OF MULTIPLE FLIGHTS FLOWN WITH A DEFERRED FUEL SYSTEM QUANTITY GAUGE; WITHOUT THE FIRST MEL REQUIREMENT PROVISOS TO ADD AND PLACARD A SECOND MEL.
Narrative: I WAS INFORMED THAT ONE OF MY FLTS WAS RELEASED WITH MEL 28-X; BUT IT DID NOT HAVE MEL 28-Y ATTACHED. PROVISOS OF FIRST MEL STATES THAT SECOND MEL WILL ALSO BE PLACARDED. I LOOK AT THE MEL BOOK FOR EACH MEL; BUT MISSED THIS PARTICULAR PROVISOS FOR ADDING THE ADDITIONAL MEL. I WILL BE MORE VIGILANT IN READING MELS. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: REPORTER STATED MAINT APPARENTLY DID NOT SEE THE REQUIREMENT UNDER THE FIRST MEL PROVISOS TO ADD AN ADDITIONAL MEL AND PLACARD PRIOR TO ACFT RELEASE. BECAUSE THIS ADDITIONAL MEL WAS NOT INPUT INTO THEIR MAINT RELEASE DATA BASE; THE ACFT FLEW MANY FLIGHTS WITH MULTIPLE LAYERS OF GROUPS; INCLUDING MECHANICS; FLIGHT CREWS AND DISPATCHERS; MISSING THE REQUIREMENT FOR A SECOND MEL PROCEDURE.
