Narrative:
Aircraft X; a B717-200; was dispatched without a cdl for a flap track cover that had been removed by maintenance. The dispatcher did not add the cdl item to the dispatch release that myself and the dispatcher signed. Item was not added to the flight release. After 6 ensuing crews had flown the aircraft one crew noted the error and reported it to maintenance control. The cdl item carried a performance penalty for takeoff and landing of 250/350 pounds. These limits were not exceeded at any time. The situation occurred because the temporary entry into the maintenance log sheet at the front of the logbook was actually referring to a similar but not exactly the same condition. Although the preflight setup of an aircraft is busy and lends itself to missing such details; I will be ensuring that this potentially reoccurring situation is not allowed to happen again.
Original NASA ASRS Text
Title: A B717 PILOT RPTS SIX FLT CREWS FLEW AN ACFT WITH A FLAP TRACK COVER REMOVED BUT THE CDL NOT IN THE MAINT LOG. THE 350 LBS WT DECREMENT WAS NOT EXCEEDED.
Narrative: ACFT X; A B717-200; WAS DISPATCHED WITHOUT A CDL FOR A FLAP TRACK COVER THAT HAD BEEN REMOVED BY MAINT. THE DISPATCHER DID NOT ADD THE CDL ITEM TO THE DISPATCH RELEASE THAT MYSELF AND THE DISPATCHER SIGNED. ITEM WAS NOT ADDED TO THE FLT RELEASE. AFTER 6 ENSUING CREWS HAD FLOWN THE ACFT ONE CREW NOTED THE ERROR AND RPTED IT TO MAINT CTL. THE CDL ITEM CARRIED A PERFORMANCE PENALTY FOR TKOF AND LNDG OF 250/350 LBS. THESE LIMITS WERE NOT EXCEEDED AT ANY TIME. THE SITUATION OCCURRED BECAUSE THE TEMPORARY ENTRY INTO THE MAINT LOG SHEET AT THE FRONT OF THE LOGBOOK WAS ACTUALLY REFERRING TO A SIMILAR BUT NOT EXACTLY THE SAME CONDITION. ALTHOUGH THE PREFLT SETUP OF AN ACFT IS BUSY AND LENDS ITSELF TO MISSING SUCH DETAILS; I WILL BE ENSURING THAT THIS POTENTIALLY REOCCURRING SITUATION IS NOT ALLOWED TO HAPPEN AGAIN.
