Narrative:
This B747 aircraft was dispatched out of ZZZZ to ZZZ with the #2 air conditioning pack inoperative. This was the 4TH leg this aircraft has flown with the #2 pack inoperative and the second time in 2 days I've flown this plane. Additionally; this aircraft has a history of pack #2 problems which maintenance seems unwilling to fix. On the ground in ZZZZ prior to boarding; the aft cabin temperature was 94 degrees F despite having run packs #1 and #3 for an hour and a half prior to boarding (ground air is unavailable in ZZZZ.) by the time boarding was complete; the aft cabin temperature was 98 degrees F with high humidity. This produces heat indexes unsafe for the elderly and the very young. We had many complaints from passenger regarding the high heat levels. This was extremely hard on them as well as the flight attendants. The following cabin temperatures (aft) were recorded: 1 hour after takeoff; 95 degrees F; 2 hours after takeoff; 84 degrees F; 3 hours after takeoff 78 degrees F. Approximately an hour after departure from ZZZZ; a flight attendant reported there was a woman in coach having trouble breathing; they had the aed out and 2 doctors were attending her. They moved her to a cooler location and the doctors continued to monitor her vitals. An hour later the passenger's symptoms resolved. The cockpit is the hottest location in the B747 and these levels of temperature take away from our level of performance. Operating the B747-400 with only 2 of the 3 packs operating in the gate/pushback/taxi mode any but the coldest of ambient conditions is unsafe due to the high and prolonged cabin temperatures produced.

Title: A B747-400 WAS DISPATCHED FROM A FO AIRPORT TO THE UNITED STATES WITH A PACK INOP; REPORTEDLY FOR THE FOURTH LEG. THE REPORTER ALLEGED CABIN AND COCKPIT TEMPERATURES WERE DANGEROUSLY HIGH.
Narrative: THIS B747 ACFT WAS DISPATCHED OUT OF ZZZZ TO ZZZ WITH THE #2 AIR CONDITIONING PACK INOP. ADDITIONALLY; THIS ACFT HAS A HISTORY OF PACK #2 PROBS WHICH MAINT SEEMS UNWILLING TO FIX. ON THE GND IN ZZZZ PRIOR TO BOARDING; THE AFT CABIN TEMP WAS 94 DEGS F DESPITE HAVING RUN PACKS #1 AND #3 FOR AN HR AND A HALF PRIOR TO BOARDING (GND AIR IS UNAVAILABLE IN ZZZZ.) THEY MOVED HER TO A COOLER LOCATION AND THE DOCTORS CONTINUED TO MONITOR HER VITALS. AN HR LATER THE PAX'S SYMPTOMS RESOLVED. THE COCKPIT IS THE HOTTEST LOCATION IN THE B747 AND THESE LEVELS OF TEMP TAKE AWAY FROM OUR LEVEL OF PERFORMANCE. OPERATING THE B747-400 WITH ONLY 2 OF THE 3 PACKS OPERATING IN THE GATE/PUSHBACK/TAXI MODE ANY BUT THE COLDEST OF AMBIENT CONDITIONS IS UNSAFE DUE TO THE HIGH AND PROLONGED CABIN TEMPS PRODUCED.
