Narrative:
Following winter storm on 2/tue/88 had aircraft deiced by XXX service, brainard field, hartford, ct, 2/sat/88. Departed brainard en route islip mac arthur field at XX00 local. Approximately 15 mins after departure while in cavu conditions entire windshield iced over reducing forward visibility significantly. Contacted ny approach, informed them of situation and requested ILS runway 24 rather than ATIS reported visual to runway 28. Subsequently landed west/O incident. Called XXXX to report incident. Maintenance supervisor speculated improper alcohol/water mixture had been used and apologized. Note: non instrument rated pilot would have had great difficulty navigating and executing approach/landing.

Narrative: FOLLOWING WINTER STORM ON 2/TUE/88 HAD ACFT DEICED BY XXX SVC, BRAINARD FIELD, HARTFORD, CT, 2/SAT/88. DEPARTED BRAINARD ENRTE ISLIP MAC ARTHUR FIELD AT XX00 LCL. APPROX 15 MINS AFTER DEP WHILE IN CAVU CONDITIONS ENTIRE WINDSHIELD ICED OVER REDUCING FORWARD VIS SIGNIFICANTLY. CONTACTED NY APCH, INFORMED THEM OF SITUATION AND REQUESTED ILS RWY 24 RATHER THAN ATIS RPTED VISUAL TO RWY 28. SUBSEQUENTLY LANDED W/O INCIDENT. CALLED XXXX TO RPT INCIDENT. MAINT SUPVR SPECULATED IMPROPER ALCOHOL/WATER MIXTURE HAD BEEN USED AND APOLOGIZED. NOTE: NON INSTRUMENT RATED PLT WOULD HAVE HAD GREAT DIFFICULTY NAVIGATING AND EXECUTING APCH/LNDG.
