Narrative:
The fueler did not notify anyone that the lanyard was not attached to the fuel cap. As best we can tell; the fueler wrapped the lanyard around the connector and forced the fuel cap on. Upon arrival; the fuel cap was only half attached. We should have lost the cap enroute. It is possible that the lanyard was attached and became detached enroute; due to improperly installed fuel cap.

Title: Fuelers failed to properly install fuel cap.
Narrative: The Fueler did not notify anyone that the lanyard was not attached to the fuel cap. As best we can tell; the Fueler wrapped the lanyard around the connector and forced the fuel cap on.
